Please read this agreement carefully before using ConnectED. By clicking on the "I agree" checkbox, you agree to become bound by the terms and conditions of this agreement. The Orthopedic & Sports Institute of the Fox Valley accepts you as a user solely based on your assent to all the terms and conditions of this agreement.

The purpose of this website is to provide secure access to medical information related to our patients Worker Compensation claims in order for you to perform the functions or services necessitated by the claim on behalf of our patients. The information provided to you would be deemed protected "Electronic Private Health Information" or "EPHI" as defined in the Standards for Privacy of Individually Identifiable Health Information set forth by the U.S. Department of Health and Human Services pursuant to the Health Insurance Portability and Accountability Act of 1996, as amended ("HIPAA") but is not restricted at this time resulting from the open Worker Compensation claims.

As a user, it is your duty to protect your password and keep the information found within ConnectED private and confidential. You are prohibited from any activity related to the medical information which would violate any law, statute ordinance or regulation. Any violations concerning the privacy of our patients must be reported to the Orthopedic & Sports Institute of the Fox Valley immediately.

It is your duty to notify us if you have a change in employment or no longer need direct access to this website. Use of this website is provided at the discretion of the Orthopedic & Sports Institute of the Fox Valley. We reserve the right to terminate any user access at any time.

Legalese

"You" means the person or company entering the Orthopedic & Sports Institute Customer Portal, hereafter referred to as "the Portal". You understand that the Portal contains confidential information of the Orthopedic & Sports Institute of the Fox Valley, hereafter referred to as "the Institute" and is governed by any existing confidentiality agreement(s) You have with the Institute.

To the extent the confidential information contained in the Portal is not covered by an existing confidentiality agreement, then You agree to the following terms and conditions:

You agree to hold in confidence all confidential information contained in the Portal, which includes any and all information of a confidential nature regarding the Institute's business affairs, including without limitation, proprietary information, customer lists, and data regarding the medical services or the Institutes methodology of delivering such service, and not to disclose such information to any person, firm or corporation without the express written permission of the Institute. You further agree that the Institutes confidential or proprietary information includes any information that You learn regarding the Institutes business or any confidential information, trade secrets or proprietary data of the Institute, its subsidiaries, affiliates, or parent company or any company affiliated with the Institute and any other party to whom the Institute may have an obligation of confidentiality, whether by written agreement or otherwise, which You learn or have access to during the use of this web site or contents herein. This includes (but is not limited to) inventions or improvements, patents or patent applications, product designs and developments, research reports, market studies and plans, literature, presentations, corporate videos, advertising and exhibition schedules, and any other confidential business information. Except as expressly authorized by the Institute in writing, disclosing, divulging or revealing the aforementioned information will cause irreparable harm to the Institute for which damages will be immeasurable. In the event of such a breach of confidentiality, the Institute shall be entitled to any and all remedies available at law or in equity that the Institute deems appropriate, including, but not limited to injunctive relief and monetary damages.

In the event of a conflict between the above terms and conditions and the terms and conditions of any existing confidentiality agreement You have with the Institute, then the above terms and conditions shall control.
